From 28 September to 2 October 2026, THCS' WP10 will organize its second Learning Week 2026: “Transforming Health and Social Care: Meaning, Models and Measurement”.

Sessions include but are not limited to:

• 29 September - Real-world Transformation – Successes, Pitfalls and Lessons Learned from THCS funded projects
• 1 October - Population Health Management as a Driver of Primary Care Transformation
